I am filling IMME 5669 Schedule A Background declaration form; however, I cannot validate the form due to mandatory given name field. I have an indian passport and have no surname written on it, just the given name. Per cic, I put my given name in the surname field and left the given name field empty. I am ok validating all other forms going this way but with IMME 5669, given name is mandatory. Cic says dont put NA. Need your suggestions here.

Sorry you are having a hard time with the imm5669 form, in the form the Surname and Given Name(s) fields are mandatory.
Your situation seem a little complex as according to the instruction guide below you are to provide the names as they are in your passport.
Write your full
family name (surname or last name) and all your
given names (first, second or more) as they appear on your passport, travel or identity document.
Don’t use initials.
According to the checklist, you are allowed to submit your spousal sponsorship application package without including your passport, which you are allowed to send to IRCC later before PR visa issuance.

Although you do not need to have a valid passport or travel document when you submit your application, you must have a valid travel
document or passport to be able to travel to Canada and to be admitted for permanent residence.
I will suggest if you actually have a Surname and Given Name(s), maybe you could complete the form(s) with your Surname and given Name(s), send the completed and validated forms to IRCC, and then apply for a new passport with your full names (Surname and Given Name(s) and hope that the passport is renewed within a few months so you could send a copy of the renewed passport data page to IRCC on time........it takes between 9 to 12 months to complete the processing of the spousal sponsorship.
Alternatively, you could apply to renew your passport to have your Surname and Given Name(s), once renewed you then apply for spousal sponsorship.
