URGENT - Questions in Qubec Immigration document filling - A-0520-AA

Hi,

Myself Kartik trying to fill the documents A-0520-AA. I have few below questions can any one help me out in knowing the answers
A-0520-AA:
1. I apply as- (Principal Applicant/Accompanying Spouse or de facto spouse)
What i have to choose here, I am planning to apply for both to me(Main applicant) and my wife(dependent)

2. Question 8:
Give the title and code of your area of training, for which you recieved a diploma ,as it appears in the list of areas of training

Asking for Title/Code

I done my undergraduation and certfied as "Bachelor in Technology" (Specialization Information Technology), and i had 6 years of work experience as Senior QA Analyst (IT field- relevant study experience).

What i have to mention for this question 8.

Question1- You ned to fill two forms, one for principle applicant and one for spouse
