Urgent question for IMM5669 Schedule A

I just got a request to fill out and send in the IMM5669 Schedule A..... my question is..... they want everywhere I lived the last 10 years..... my ex husband rented an apartment in Virginia for 2 years where I use to stay while I did volunteer work with a non profit but.... I kept me NC address as my address. I did get mail there from time to time but I never changed my legal address and returned to NC permanently to my legal address once my volunteer work was over. Do I list this address (it was in 2009 when I no longer would go up and stay there for weeks at a time) or do I put my legal address that was on my drivers license.... I was registered to vote in NC and never got a Virginia tags... or taxes. I need to know ASAP please. Thanks!!!

You should provide all the addresses where you have lived in the past 10 years since your 18th birthday.....which includes the apartment in Virginia for 2 years and later the apartment in the NC address.......You were lived at NC, then temporarily at Viginia, and then moved back to NC.......put the address chronologically in the form.
