Urgent question about medical form

Hi all,

I have emdical form given by the physician on the same day of my medicals.( 1 page)

However, today the physician send me the same form but with all details of test results mentioned on it ( xray , blood test and physical examination in detail) . This form is about 4 pages long

I am confused which one to upload ? The physican said they already submitted results through their sysyten but dont know what i need to submit.

Any help pls

Both include the number CIC needs. I uploaded both, but I would imagine you could upload either one. CIC can retrieve the results either way I would think.

I only uploaded the shorter, one page information sheet and I was fine. I was never even given a detailed sheet.

Contact the doctor that did the exam. Mine emailed a copy to me the same day they sent it to CIC.

contacted his office and they told me UMI no alone is fine!

My test was in November. I already passed medical but need it for archiving and filling all my papers
