Urgent - IMM 5475 PART 3

Hello all,

I uploaded my documents for PR after receiving an ITA and I got a response to fill the IMM5475 form. I have filled it but I am not sure what to enter for the following:
1. Name of the office where the application was submitted
2. Location of office
3. Type of application: (I presume is permanent residence)

I hold an Indian Passport and have done and express entry profile online from Toronto, Ontario.
I read through a thread and I found that I should send an email to "CPC-CTD-Ottawa@cic.gc.ca"
I sent them an email but their automated response indicates that they will reply in 20 business days. Its been 4 days and I am worried at the moment.

If anyone was or is in the same situation and has an answer to the about question, I would really appreciate if they can share it with me so I can continue with my application.

PS - I only have till Sunday to respond and upload this form

Did you go through an agent? Looks like your main application might have some errors regarding this issue. Which is why they want to know where did you submit the application.

I had same request, the location of the office is not really important as much as the other important details.. when I filled mine I just wrote cic for the name and online for the office location...
it was later that I got to know my visa office...
so my advice for you is to fill CIC as name of office, and your visa office if you know it as the location... if you don't just write "online " just like me))
 
I'm in the same scenario. They requested me this form, but i completed it myself. Should I put it for my spouse, maybe?
