URGENT help please

Hi,
I was just doing my final check of our common law application when I noticed by boyfriend wrote my middle name in the Application to Sponsor, Sponsorship Agreement and Undertaking form. It says it needs to be exactly as shown on passport or travel document. I do not include my middle name in any travel documents. Should I wait and send it to Canada for my boyfriend to sign again or should I write a note explaining why happened and include my birth certificate with my full name on it?
Any help is REALLY appreciated

Well, you have to include your birth certificate in any case.

Thanks for your reply. I already have my birth certificate in the application. What I want to know if that would work as a reason why my boyfriend wrote my middle name in the application even though I don't use it on my passport. It needs to be exactly as it is on my travel documents.

Most likely, still I would re-do the forms to comply with the way they stated for you to do it.
