URGENT HELP PLEASE - Barcode!!!!!

I filled on my computer the apllication IMM 1294 and want to send it by paper.

When I click button VALIDATE, everything was ok, and then PDF put the barcodes on 6th page...

Now I have a 6 pages, on the 4th page is sing court and date, and on the 6th page is five barcodes...

(On Cic.gic website I saw that the barcodes have to be shown on 5th of 5 pages)

What to do now?
Please help me!!!

Bro actually 5th page is the part of 4th page but according to printer page setting some time its goes to 5th page so don't worry about this issue.The main thing is you have to fill the application form with exact accurate and correct information.

you have to print page 6 two copies

You can download and fill a new one to be on the safe side, although there is no cause for alarm.

Also, the page with barcode ought not to have the signature or date on it. Date and signature are meant to be on the penultimate page, which ordinarily is the 5th page.
