My AOR of eAPR is April 5th 2017. I just got an ADR for spouses proof of travel ( travel history ) and IMM5562E ( which for some reason I couldn't download the form ) .. half of my husbands travel history is in a lost passport in which I have uploaded the page in his current passport with the stamp that says that he lost his previous passport and got that stamp translated too but I haven't actually mentioned this point in the LOE .. my question is : how do I prove his travel history and what is this IMM5562E form and where do I get it from as the link provided in the request letter is not working ? Also, how do I upload these forms once filled ?
