"URGENT" Copy of long form birth certificate

Hello every one I just got email from immigration office after informing him that they has missed the medical for by older son

the Email content is just below


Dear Sir/Madam,

Upon review of your file I have noticed that we do not have supporting documents for Rayan Masood. Also note that no fees has been paid for him. Therefore we require the following

- Copy of long form birth certificate for Rayan
- Passport copy( only the bio page)
- $150 CAD bank draft

Once received we will review your file and send the remaining medical forms.

We look forward to hearing from you within 30 days.

My question is where can i find this long form??????
Does it mean the normal birth certificate which we obtained from hospital????

If you are based in Pakistan, then you need to get the NADRA birth certificate, by going to your union council office.

If you are in another country, then it might be better if you post in one of the post started for that country.

Just wondering, did you not notice that your son's fees was not paid?

Thanks for the reply, Yes I am from Pakistan.

Actually my wife was doing all there procedures since I was in Saudi Arabia and after the birth of my older son she also made draft for fee, she also filled the form and posted it via courier but she missed the original draft and just sent the copy of that.
Now after 2 years when we asked to do our medicals we noticed that they didn't sent us medical for the older son.
So we contact them by email and in reply they sent us this email.
Meanwhile the fee and form for my younger daughter was sent properly.
