UPFRONT MEDICAL..PLEASE ADVISE

Hello friends,
i have read in this forum about many people doing upfront medical.Can anyone tell me what and all documents needed to do that?Also,UCI and Application number will work for it if we don't have MR request.When I asked in a clinic they advised me to confirm with somebody regarding this….Kindly please advise the procedure….Am aware of the risks in advance medical exam….Thanks
