Upfront medical for paper based PNP

Hello.
I have a question about medicals.
I'm going to do upfront medical test, but doctor said that there are three options.
1. For Express Entry.
2. For study permit.
3. For work permit.
No upfront for paper based PR applications.
Which one should I choose, if I want medicals for pnp paper based application?

Change the doctor as he is unaware. Otherwise ask him to contact IRCC as its also allowed for paperbased applicants.

Unfortunately in my country there are only two doctors, who can do these tests. So changing is not an option.

Is there any outland who did the upfront medical exam and really make the difference regarding the processing time? is there any clue?
