upfront medical exam

Hi guys

I had a few question about giving an upfront medical exam:

1. I read that my doctor should give me some paper after I complete that I should submit with my application. What should that paper look like?

2. I already applied for a study permit but I was refused. I now have a UCI though, should I give this UCI to my doctor?

3. In the questionnaire you have to fill out before you get a document checklist, they ask you whether or not you have taken a medical exam within the last 12 months. I haven't taken a medical exam in the past 12 months yet but I will have taken it in a few days, so can I just say that I have already taken a medical and submit the questionnaire or should I just wait until after I complete my medical exam and then submit the questionnaire?
