Unable to upload medical document

Hello, I am applying for a student visa in Canada. In the IRCC Questionaire_IRCC are the answers I have given. My admission is a fully funded position at western University in canada.
My tuition fees are also paid by my supervisor.
After choosing those answers, I am unable to upload the medical document. I cant find a column to do so. The document checklist is as shown.
Please help me, how can I upload my medical document in this case.

Yes, that is waht I did. Attached it under the client information. All the documents that I was unable to upload in specific columns like transcripts, IELTS scorecard etc.

When going through the questionnaire, did you indicate that you have already completed a medical exam in the last 12 months? If you did, it should give you a slot for "proof of medical exam".

Yes, I indicated yes in that but still I didn't get a tab for uploading the medical proof. You can go through the questions I answered in my main question post it is linked to the image of my answered questions.
