additional two test required namely HBsAg and ANTI-HCV test ,ANTI-HCV test is 1,860 pesos while HBsAg test cost 905 pesos. No
appointments required it is on first come first serve basis. Thats why it is more better if you arrive in St. Lukes as early as 7:00 am.

Work Flow Steps
1.Canadian Reception (3rd flr.) Medical requirements:
1.1 Submission of requirements 1.Photocopy of passport( front only)-2 copy
1.2Filling-out the applicants (PDS- Patient Data Sheets) 2. 6 pcs. of passport size picture (white or blue background)
1.3Photo encoding and Patient data encoding 3. photocopy of MR (Form IMM1017)
2.Cashier (5th flr.) 4. photocopy of 2 valid ID (example SSS, Drivers license, PRC ID,
Payment of fees voters ID, etc.)- 2 copy each
3.X-ray (3rd flr.)
4.Laboratory(4th flr.)
4.1 take sample of urine and blood
5.Vital signs table(5th flr.)
5.1check blood pressure
5.2check height and weight
5.3 check visual acuity
6. US counter (for room assignment of physical exam.)
7.Physical exam.( all applicants are asked to disrobe completely)
8.Canadian Reception (wait for Official reciept)
Finished (according to them wait for 3 working days for possible call from St. Lukes if their is a problem for the medical or required
another test, then if their is no call from them, it means that medical examination is ok and it is automatically delivered to CEM)

Kung ako po ang nasa kalagayan mo It has already morethan 2months po kaya sobra na po sa timeframe ng CEM na 1-2months
lng dapat may MR na. Pwede po kayong magfollow-up kung gusto nyo.basta pumili lng po ng isang way ng communication ninyo either
through e-mail, fax or call .Ito po ang mga contacts nila kung gusto po ninyo magfollow-up. Please mention your tracking number,name of courier and when did you send your application.Don't worry po it will not affect your application marami na pong nagfollow-up sa application nila na naapproved naman ang visa nila.i just wondering may be your application were not received by CEM wag naman po sana.Sabi nga po ng CEM to fast track your application they advice to use their official courier which is air21. Sa palagay ko isa yun sa reason kung bakit until now wala ka pang MR. Kasi nga po LBC ang courier ninyo but anyway i will not blame you
