Hello. I’m applying visitor visa for my mother & sister. I was wondering do I have to make two different profiles/ 2 gc key accounts? Please guide thank you

One group and 2 applications underneath it . When you login to IRCC portal it will ask you that to specify.

Also just don't blindly apply. Ensure you have provided enough documentation to prove family ties, assets, income statements under the applicant's name and not yours, length of stay keep short ( say 1-2 weeks), and funds for the same . If sisters and mothers have properties and jobs give details about that. Stupid immigration officers may reject without even realizing how unique one's situation can be.
Do a 2-3 times audit and do enough research to see that you have provided required additional documents.

oh yes tell me about it. I did my whole PR/Express entry process by myself and I believe that’s easier than the visit visa documentation it took us months to gather everything
Do you think adding a pregnancy emotional letter from midwife is a good idea? Some people suggest doing it some people totally say not to do it.

Not if the sister is over 22.
so does that mean two separate accounts? How am I going to explain CIC that they’re both applying together & plan to travel together if approved?

Is she over 22, then yes. They both need to show their own funds, travel history and ties to home country. It can be stated in each application that they plan to travel with a family member. Don’t mention mention the pregnancy as it may be seen as tie to not return and ”help” around the house.
