TRV Extension doubt

Hi,

I have landed in Canada as a student on May 2013 and am currently on a work permit that is valid till end of 2015.

My visa expires on Aug 30th 2014. I am trying to apply for TRV by filling the form IMM5275. Am I filling the right form ?

While filling the form, there is a question about details of visit with options as Student, Business, Returning student, visitor, returning worker categories. Should I choose the option of returning worker ?

I have not travelled back to my home country after coming in May 2013. I am currently working in Canada.
