When I am reapplying do I need to mention the UCI number once again.

Is there any sample of affidavit of financial support from parents available

As this case is complex, you may think this is the time to seek legal advice for help with the application and affidavit. Again, I am not sure that a promise from your parents to give or lend you money will be perceived by the VO as a tie to your home country, even though it does show strength of your family ties and support.
