Trigger to count application against CAP

I understand that there will be cap for application/NOC. I understand it is by first come first served basis. What is the criteria immigation officials follow to decide first come to allot a position against cap? It is when we submit our application online? or it is when our physical application reaches? or when some one reviews our application and accepts it?

How applicant will now that his application is considered against the cap?
