For travel history, you would be entering this in the TRV application form, not sure why you need a template for this.

They have considered below points for refusing my visa.

What documents did you submit last time? Your travel centenary can be a simple document that explains a day to day schedule. For example:

Day 1: Niagara falls.
Day 2: CN Tower and Ontario Science Museum

However, this won't help you in your other 2 reasons for refusal. Are you unemployed? You also need to show ties to where you live. Are you an expat living outside of your country of citizenship?

You can include per day expense estimates for travel, sightseeing, meals, etc + hotel (if you do not have a host who is inviting you)..... and prove that you have the funds for this itinerary
