Thanks for the heads up, but I don't see Ottawa mentioned on my forms or email, so I'm guessing mine will be sent to Sydney?!

Also, did everyone else get their medical forms attached as an image file (.png)? I've just gone to print mine now and I've had to scale it down to fit it onto paper and it gets blurry. Very annoying, especially as the instructions are attached as a word document, therefore far easier to print! ::)

Yap mine as well got image file ;) Your medical result will be send depends on what VO written in your medical form on the upper side area at the right side. ;)

The mailing address should be found in your medical instruction.
My case, the form does have CIO as an office but the instruction says “medical reports and X-ray must be sent by the Panel Physician to RNH-Ottawa”.
My doctor seems to send the documents automatically to Ottawa since he told me to do so without looking my instruction. Now there was a case who got Sydney instead, so I just thought it might be worth to do double check with your doctor where to be sent.

Mine is also from CIO. And the form or email did not mention Ottawa at all. There is no address for CIO on the form. I assume that the doctor knows the exact address.

OK. As far as I go through the DMP handbook, all medical reports prepared in Canada go to one location, Laurier Ave. West, Ottawa. No CIO Sydney address was found on the list (total 8 locations in the world). This address has been used for long time (at least for 6 years) so the all DMPs should be used to it. We just got instruction in different format, I guess.

I thought everyone would get the same email and form requesting for MR. I re-looked at my email. What I understand is that the CIO-Sydney is responsible for processing my application. And it also says that there will be a partner Visa Office to collect permanent resident rights fee something like that. Anyway, the address to send the MR report should be known by the doctor. I won't bother that.

Is anyone seeing that there is a sentence like "let CIO know, an Approval of an Arranged Employment Opinion by Human Resources and Skills Development Canada". What does that mean though?
 
Just now got the news from bank that my bank draft got encashed yesterday.. hopefully.. AOR will follow soon... :D :D

it seems that you and my brother has the same date when CC has been charged. I'm his representative here, I wonder if AOR (from CIO) will be sent through email or mail and to whom (applicant or representative).

AOR will be sent through email.....i dont know about the representative as I was the principle applicant in my case with no representative
