I am applying online but I need to submit supporting documents that will be scanned. I think I need a marriage certificate but I have not looked into the process fully or which documents are required.

OK, I just checked my online application. My checklist asked for:
1. Application to Change Conditions, Extend my Stay or Remain in Canada as a Visitor or a Temporary Resident Permit Holder (IMM5708) (required)
2. Family Member Proof of Status (required)
3. Proof of Means of Financial Support (required)
4. Proof of Medical Exam (required)
5. Passport or travel document (required)
6. Digital Photo (required)
7. Letter of Explanation (optional)
8. 100 CAD fee (required)

My advice to you is: go to http://onlineservices-servicesenligne.cic.gc.ca/eapp/eapp.do
Do the questionnaire and save the checklist code you get!

If you don't have a myCIC account, make one here: http://www.cic.gc.ca/english/e-services/mycic.asp
I used GCkey, but I guess you choose the one that is better for you.

Enter the checklist you got from the questionnaire to your account, and then start gathering docs and filling the form. You don't have to upload it all at once, so you can see what you need and do it one thing at a time.

As far as I know, the checklist you are mentioning is used only if you are filing you application on paper.

No problem. I always panic, even after I uploaded everything and applied for extension, I still re-checked everything 5 times. When you come home, go through everything and you will feel better. If you have any questions afterwards, then ask again here (or PM me), but better here, because then more members can help you. But first go through all of it yourself, it is pretty straightforward. I gave you some general guidelines :) Good luck!

Yes, I am well aware of where to find this information.

You stated that a medical was only required when applying online and not when applying on paper. I was clarifying in what situation a medical is actually required and that the requirement doesn't change depending on how a person applies. If a person requiring a medical applies on paper, CIC will approve the app contingent on them completing a medical within 30 days.

There is no need for another medical. The receipt from the PR medical can be used for the visitor extension app.
 
Sorry, this must have been a misunderstanding, you don't need to get angry. I was just trying to help, maybe I didn't give all the information, but that's why more than one person replies, so that we can fill in what others have missed.

Angry? I didn't use CAPS, exclamation marks, bold. I simply made a statement clarifying why I made my original post and what it meant, so I'm not quite sure why you inferred anger from that.
