From my understanding the medical required for express entry is the upfront medical. You should be fine.

Depends if they use e-medical or not. One doctor in Toronto that used e-medical required: passport, passport sized photo, express entry profile number, $280 cash. Once it was completed they submitted everything electronically and they give you an up-front medical notification printout which you scan and upload once you get an ITA.

If the doctor paper files then 4 passport photos are required. Be sure to bring your prescription eye glasses if you require them.

In Toronto I recommend this guy: http://www.visadoctor.ca/canada

Be sure to read over this before you go: http://www.cic.gc.ca/english/information/medical/medexams-perm.asp

It says you only need passport photos if they file by paper mail but I have seen eMedical doctors requiring 1 passport photo for their own records.
