I unfortunately don't have concrete answers to your questions... Sometimes the visa office requests medicals, RPRF and PP all together, sometimes separately... from what I've read I guess it's case-by-case. In my case, I've been waiting for 4 years without much news from Manila but provided updates a few times, including a thorough one to CIC Tokyo last year, plus my husband is a Canadian citizen, so maybe they just concluded that I have enough to get the visa. But still, I don't know when the visa will be stamped... that's the million dollar question! With such a new visa office it's hard to say. I'm just hoping they do mine in one month or so.
So in your medical instructions you don't have the line saying "Medical results were sent to Manila on:... " ? And you don't have any other instructions? Perhaps you have to check with the visa office on that one.
About the medical exam, I did it at the Tokyo Medical & Surgical clinic. The X-ray is just for the chest, and they do give you a protective shield to put on your waist. At the reception you'll get some forms to fill out, and I think in one of them they ask if you're pregnant. Also the doctor who checks you up will ask this question.
I really hope everything goes well and you can get the medicals done without hassles!
