hello oo nag memetformin din ako actually mag one yr na nga ako nag tatake ng metformin once a day lang naman then sabi ng dra. ko another 1 yr pa hangang pumayat ako! hirap pumayat sa pcos pero kailangan mag papayat para maiwasan yung complication! sabi niya kung mag mimigrate nga daw ako for sure within 5 yrs mag kaka diabetes ako! nakakatakot din ako, siguro healthy lifestyle nalang para maiwasan 32yrs old plang ako then gusto ko din magkaanak. check mo nalang palagi cholesterol at fbs mo then khit once a yr mag pa HBA1C ka para maagapan na kung mag develop man......keep in touch nalang tnx
 
Nagpa meds kami last Dec 1 sa Nationwide. Eto kwento ko nangyari.

To save time:
1. Magdala ng madaming ballpen
2. Dalhin lahat ng medical certs kung applicable, previous xray kung me previous case ng PTB or exposure sa PTB
3. Kung nagkaperiod ka ( female ) make sure na pumunta ka after 7 days ng iyong last day
4. Mag pa picture na sa labas, 4x passport size lang kelangan, meron naman sa 4th floor kaso hintayin mo pa.
5. Hawakan ang inyong passport at yung ibibigay na checklist sa step 3 at all time
6. Passport Copy
7. Pictures with red background not acceptable


Eto step-by-step

1. Punta sa counter kuha ng forms
2. Fill up ng sangkatutak na forms at ipaste ang mga pictures, i sign na rin ang pictures
3. Punta sa table 2 para ma assess ang forms, bibigyan ka ng ID at checklist
4. Hintayin tawagin para magbayad
P4,000 15-above
P2,000 11-14yrs old
P1,750 11 below
P50 additional fee for courier service

5. Eye checkup, weight, height
6. Interview ng doc, undress para ma checkup ( CPE )
7. Xray
8. Urine at Blood
9. Tatawagin ka sa table 3 kung ok na tapos uwi ka na
10. Bigay ang checklist at id sa lobby

Sana makatulong.

Medyo mahirap pala hanapin ang building kasi nakatakip ang bldg number. Hanapin nyo na lang slimmers world, tabi lang non.

i just want to ask something... why is it the Nationwide Health System Inc. always asking to take a chest xray it happens to my father.. kasi ilang beses na sya pinapapbalik para lang sa chest xray.. taz ndi nman nila sinasabi kung masakit ang tatay ko,, ang gara nila :(. 2 years na kami nagaaply still ndi pa din namin nakkuha visa namin.. dba pag may problema naman sinasabi nila para magamot? eh bat ung sa tatay ko wala naman sila sinasabi,, aizxt maeexpire na ang visa namen hanggang ngaun nasa embassy pa den. ayaw ibigay dahil lng sa medical. :( ..
i notice that im the only person who is speaking in tagalog.. sorry because im not fluent in speaking in english thats why :D

hi mz_aleli16. ok lang naman mag tagalog at mga pinoy naman ang nandito. anyway, naitanong mo ba kung bakit nag-pa-repeat xray ang father mo? usually, may nakikita sila like scars or something. now, kung suspect for PTB then minsan pina-pa-additional test pa like sputum test. tanungin nyo yung nurse or DMP kung bakit pina-pa-repeat xray para may course of action na rin kayo. pwede rin, mag pa-test kayo sa sarili nyo just to ensure of your father's health.

Hello guys im also one of those waiting for MR. just want to ask po, i just have my mamogram with breast ultrasound last march and i have a 5 cyst sa aking breast. pero hindi naman daw cancerous. last CY 2008 i undergo mamogram and wala po ako cyst. last march po ay ayun 5 na. meaning mukhan dadami pa ata kc in 2 and a half years may 5 na kaagad. then meron din ako myoma na 3 and polycstic din ako. hasy dami nuh. ewan ko ba. maried po pala ako pero no kids pa din after 6 yrs.

ang tanung ko is ideclare ko ba un halimbawa dun sa form na fifil upon?

Question: My son who is 7yrs old got a surgery (elbow) at the orthopedic last year (out patient) but he need to make a follow up check-up for the last 6 weeks then undergo therapy for a month but now his elbow is back to normal. My question is do we still need to get a medical certificate on his case, before going to response to his medical request examination by next week. Thank you.
