To All Who Made Medical Examination to Nationwide Health System Inc.

Ask ko lang what are the things i need to bring sa medical exam. I called nationwide health and they mentioned to bring a letter coming from the embassy, ito ba yung IMM 1017? My hubby just emailed me and told me that the papers send back to him and requesting for our medical. Pls help, I am planning to do my medical any time this week. pls help

a letter requesting your medical examination
medical report or IMM1017 ung my picture nyo
passport
previous medical reports (such as confinements etc.) if any
4 recent passport sized pictures
eyeglasses if your wearing one
vaccinations ng mga anak mo
payment

I dont have letter of request for medical examination. My hubby passed our documents in Canada and they just send it back to him telling that I have to do my medical exam.Thats it. Wala silang binigay na letter of request. Please help. Im getiing confuse.

paprint mo n lng yung appendix c ata yun basta yung sa application form ..meron nun eh ..under spousal sponsorship ....bgo ko isubmit yung sa akin ngpamedical n ko eh...yun lng dinala ko passport at yung appendix c ..photos... ;D

my letter ung kasama..nakalagay dun kung saan ka pede magpa medical..kung what ang kailanagan mong dalhin.kung magkano babayaran mo..nakastamp din sa front nun ang date kung kailan nla pinadala @ kung anong embassy..nkalagay din sa letter na un na hanggang w/in 30 days upon reciept of the letter ka pde magpamedical.

ang letter of medical request @ ang IMM1017 ay makasamang binibigay or pinapadala ng embassy.
kaya hndi ka pede magkaroon ng IMM1017 kung wala kang request for medical.i hope this could help you.

kelan po kayo ngpamedical kami po nung Jul. 21 sa Nationwide, nagfollow up po ako ang sabi it was submitted July 29 ung result ko, then my husband's result Aug. 2.

june 18 kami ngpamedical @ pnsa ng Nationwide ng june 28 sa embassy.ung husband ko kc sa calgary na ngpamedical.kya no idea ako kaialn ipinasa ung skanya..tnx

Guys, sino magpamedical this Sept? please patanong naman sa nationawide if ok lang na ang dala ng mga bata going doon ay copy ng kanilang passport...nandito kasi sa kin pasport nila sa GCC. thanks in advance, kung sino po makasagot sa akin...God bless po sa medical.di ko kasi macontack yong phone nila.

strict po pagdating sa requirements ang nationwide, yung anak ko dala ko noon yung birth cert nya, pero d nila tinanggap. Pero ang alam ko pwede kahit passport copy lang. Sana may makatulong sa tanong mo po,,=(

please help me!!!!! . We did our medical and DMP submitted it to Ottawa last July 2010. Until now we don't hear anything about out PPR. and Ecas says still in process.anyone who have the same situation. thanks please help

Anyone here who had their application processed in CIC's Visa office in Buffalo, NY?

My husband submitted our application in Buffalo since he's already in Canada. Just wondering ilang months before i-request ang passport for visa stamping after ma-submit ang medical exam sa Ottawa? Is there a way to check the status sa Ottawa? Hirap kasi they discourage doing follow ups within the normal processing time, kaya need your feedback para may idea ko until when kami maghihintay for a decision to be made. We applied under SINP, timeline namin below:

Feb. 2010 - submitted SINP
March 2010 - SINP approved and sent Application for Permanent Residence to Buffalo,NY
June 23 2010 - received request for Medical Exam from Buffalo, NY
August 5 2010 - medical exam results received at Ottawa (as per Nationwide's records)
