No you can go to one in Canada.

Also, it looks like I have to take a medical exam by a Panal Physician in my would-be country of residence (the United States) and I (like I have stated before) cannot simply take off work to do this. I do not see an option to see one of these doctors in Canada. How do you get around this?
Several people here have applied outland while living in Canada, and some for long periods of time. There is nothing wrong with it, and the inland guide even recommends it.

You do not have to go to the US for a medical, you can do it in Canada. Just look at the options on the site. There are 11 doctors in Montreal, so you've got quite the selection.

Thousands of people have done it with no problem.

If you read the CIC guide on inland process, they actually encourage people to apply outland if they can.

The family class is aimed at faster processing of spouse or common‑law partner applications from applicants outside Canada.

Processing times for spouses or common‑law partners in Canada are generally longer.

The person being sponsored can apply to a visa office outside Canada to take advantage of the family class processing standard. Processing times for applications processed in Canada are posted on our Web site.

You fill out ONLY the outside-Canada application package.

Entire app is sent to CPC-Mississauga. After sponsor approval CPC-M will then send it on to the appropriate outland visa office.

They should provide you everything you need (though I think you're supposed to take in a few photos... check your guide). Make sure to tell them it's an "upfront" medical exam. After the exam is finished you should get a receipt with a reference number on it, that you need to include in your app.
