The Medicals are valid for one year - and usually the landing must be done within this period itself. The visa validity is also usually noted likewise for the same period - i.e. one year from the medicals.

visa expires exactly a year after the date of your medical exam. had ours last august 18, 2010 and visa expires august 19, 2011.

This would depend on the charges from the authorised doctor (called DMP by CIC) that you go to and it definitely differs from country to country. But if it helps, in U.A.E., Dubai, the clinic we went to charged us as below (the cost here in Dubai is much higher than in my home country):
Per adult - AED 750 (roughly around USD 200)
Per child (I think up to 12 years or so) - AED 350 (roughly USD 100)

In Australia the fees for medical are as follows:
Adult : Med Exam : 209 AUD
X-Ray : 110 AUD
Blood test : 100 AUD

ChildBelow 11 years)
: Med Exam : 209 AUD

However, it may varies between countries and DMP i guess
