For those who worry about last year translations - I used all documents translated and certified from last year application. Except my mom`s passport (were changes) and my citizenship certificate. So far it`s accepted. Hope this helps to calm you down, guys))

Also Fyi...my observation based on the information few members already shared in this forum...I was constantly tracking Puralator website (every 5-10 mins) on 4th Jan, and noticed that they updated the status online @ 2:00 pm on 4th Jan for my app...(approx the same time when the Puralator van left CIC M campus)...hence not sure how Puralator mentioned that the delivery was done at 9:43 AM...and what was the approx time they actually submitted the docs.

If you go back a few pages you will see that we discussed how Purolator used smaller companies for the 9am delivery and therefore that the 2pm truck was probably only there for same day delivery.

I used this form was downloaded in late Nov, 2015, Form CODE: IMM 5406 (09-2013) E.

By the time, It might be PDF Adobe's issue or else, when open the form on computer, the form has "Dropdown" for some sections/boxes, such as" Place of Birth " "Marital Status" even the "Address",

After completed form, printed it out, It shows very tiny square black dot with white cross in the tiny dot at the right bottom of corner of the each box.

will it be a problem to concern about "Wrong Form " ? I used the form has the code IMM5406 (09-2013 )E, is still the same as last day on CIC website , the other info/layout and so on are the same as I checked/compared many times.. no different at all.

Family Information form is just for information, this is not the main form, if all the information is available, I don't think any issues with the formatting or anything, also the form number is same you used

Thanks a lot. how about the "Place of Birth " It's very frustrating, it's like "Toronto, Ontario, Canada " or Toronto, Canada" or "Ontario, Canada" ... you know what I meant?

The main form IMM0008 I wrote City/Town (form asks) and province only because the passport shows the name is her country's province. I don't want to CIC think it's different info compare the supporting doc and the form info I wrote,

But in IMM 5406, I wrote "City, Country" because I think it should match the name of the city from main form I filled as CIC required.

However, put "City, Country " in "Place of Birth " on IMM 5406 might not match the passport.

Very stressful.. any experiences or thoughts on that? thanks a ton !!!

5000 * $200 = $1,000,000 will be winning lottery for them just because CIC is fool enough to not implement a streamlined method of accepting applications.

Agree, that's why it's important to look for a local courier who can deliver your docs before big couriers and with relative good price and outstanding service.

Last year, they accepts way over 7K+

Just give it some time. I told clients the range of applications they were in. If you want the copy of the stamp please email me with the tracking number in the subject line and I will send it to you now and if you want to know what time I was there you can ask Ken at happy/3xpress or Lenox at VW. We had plenty of time to talk and get familiar with one another. They know who I am. Actually the numner should include all of us with in an approximation.

I trust what Chris said. It makes sense. Folks, if you used local couriers other than HR please be patient. Right now most of the people who got charged are from HR as they have 700 docs in; and VW, Metro Mississauga and Deal Made Express(3xpress) and others are the local couriers with over 100+ docs delivered after HR but before 9am on Jan 04. The ones who used these couriers please expect to be charged very soon, probably within next week.

Thank you for sharing this. I used my birth certificate translation from summer of 2014 and this info is helpful to me.
