ung UCI po nakalagay po un sa Work permit mo, parang SIN din po yan when processing or question about your application they will ask for your UCI # kaya importante na ilagay nyo po un...

Sabi nga, kung first time mo ito (i.e. hindi ka pa nag apply ng work permit, or study or temporary resident visa) then wala ka pa nito.
Makukuha mo ito sa AoR kasama ng file number mo

Note - rule of thumb ko dito is check kung me instructions sa itaas ng form pertaining sa paggamit ng Not Applicable or NA. Merong mga forms na me specific instructions ano ang ilalagay. So if ever makatagpo ka ng ganito:
= follow what the form says
= if no specific instructions - follow what the guide says

Me dalawang areas dyan.
First question is - pumapayag ka ba na ma-contact ng CIC at tanungin ukol sa services na ibinigay nila? Remember for research etc. purposes.
- collection ng information ito ng Federal agency (CIC)
- isipin mo na lang na isa itong customer survey
- pwede ka sumagot ng No, when in doubt, sagot Yes (the normal polite way...hehehe)
- pwede ka naman mag-opt-out later kapag nasa Canada ka na - every message from CIC tungkol sa info eh me opportunity to opt out
- me batas kasi sa personal information sharing dito - kaya kailangan parati ng consent. And sa forms ng CIC, me personal information ka. Also, pwede ipasok ito under anti-spamming laws kaya hinihingi ang consent na gamitin ang personal info mo para makontak ka.

2nd question is me two parts:
- pumapayag ka ba na ang personal information mo ay pwede ipamahagi ng CIC sa province na nag nominate sa iyo/destination mo
- pumapayag ka ba na ipamahagi din ang information mo sa prospective employers (i.e. pwede ibigay as part ng background check..)
So:
- collection ng information ito ng Province agencies naman (pwede Provincial Nominee program or Province Labor commission)
- pwede ka sumagot ng No, when in doubt, sagot Yes (the normal polite way...hehehe)
- pwede ka naman mag-opt-out later kapag nasa Canada ka na - every message from CIC tungkol sa info eh me opportunity to opt out

Name na ilalagay - ikaw ang applicant (i.e. principal applicant) - so name mo...ikaw ang pipirma nito. Ang spouse mo and children - dependents.

Kaya nakalagay sa statement:
I (first name, last name)_________________________ on behalf of myself and all dependants included in this application...
