Guys, sinong may copy sa inyo ng Appendix A na kailangan naka attached along with passport when submitting to embassy? Kasi the embassy called me last week to submit my passport with photographs without telling me na kailangan pala kasama pala ang appendix A and they did not even send to me the appendix A form. So, i sent my passport last week but they called me yeasterday, they asking about the appendix A form. I asked them were can i found the appendix A form the call center agent just answer me na nasa website lang daw nila. Then i checked their website also the cic website pero hndi ko makita ung sinasabi nila na forms.
I need your help guys, especially seniors if saan ko ito makuha na forms. Thanks
