NOTICE – IMMIGRATION MEDICAL EXAMINATIONS PERFORMED

Dear Sir/Madam:

Good day. We would like to inform you that the Immigration Medical Examinations (IME) have been performed by Designated Medical Practitioners (DMPs) for both me and my <spouse/dependents> – <name of Principal Applicant> and <name of spouse/dependents>

This is in fulfillment of the requirements as specified in the Medical Examination Instructions for Immigrants, sent from the Embassy of Canada in Manila stamped <date>, postmarked <date>, Makati City Post Office (MCPO), Makati City, Philippines and received at our residence in <town/country> on <date received>.

• 
  IME for <Principal Applicant - or if PA and Dependents had the meds done on the same date and with the same IME - you can just list all the names here> was performed on the <date> and done by <Full name of DMP> at his/her clinic at <full address of clinic>.
• <you can repeat the line above for each dependent who did the meds separately - just update all the details - date, addresses>

Please find attached copies of the receipts for these IMEs performed by the respective DMPs.

• Receipts for IME for <PA Name> – for One (1) Full Adult Medical, Receipt Nos. <#> and <#> both dated <date of receipt>
• <you can repeat the line above for each dependent who did the meds separately - just update all the receipt details - date, addresses>

We trust these are in compliance with the instructions detailed in the above-mentioned document (Medical Examination Instructions for Immigrants) .

IMM5620 is the form for payment of application processing fees, not for RPRF. So far there is no official form yet for payment of RPRF. You can just attach the payment check to a letter and specify that this is for payment of RPRF (include details such as number of persons included in the payment). Or as what mayonnaise did - print out the email or in a copy of the letter from CEM, attach the check or payment.

Better use the 1st address - when sending the payment. Are you just sending the payment by itself or the payment is included among other documents that you will submit to CEM? If you still have documents to submit to CEM, you can just include also the RPRF payment so you can courier all at the same time.

pumunta po kaming embassy yesterday for fingerprinting para sa NBI clearance. then papadala namin sa kamag-anak sa atin para file nila.

hindi ako nakapagtanong regarding sa passport. dami kasing tao nun. will try to call them sometime this week.

pinakuha din po ba kayo nga clearance from SG Police Force? ganu po katagal bago nyo nakuha?

Malapit na po ang PPR nyo kasi doon sa mga nag IN process na sa e-cas a week lang may PPR na sila.
