What do you mean? How can I spot the difference and how will you differentiate the two the same forms but not the content? What is the detailed explanation? Thank you
 
The only thing different is IME#.I did get the forms yet but somebody told me that they were dofferet.compare for yourself.take a look at the old one and at new one

i will share my situation here.
sent file in sydney under sinp: april 17 /12
medical request: end of may/12
medical done: june 15/12
status: till now medical received
redone medical upon request for dependents: april 30/13( not for me been extended for another year)
documents request:
1. uci no. and my name
2. rprf by may 30/13
i am not sure what its mean by point 1. it says all documents must me sent to our office by may 30. if unable to send explain.

medical redone may 01/13, rprf as well. status still same till today.
