Hi Scylla,

Thanks soo much for the help! It all makes sense now. Now I have another question. When we called the doctor to book the appointment, her secy asked us to bring the form. So am thinking we only fill out the form from appendix C and take it with us and the DMP will have IMM 1017 EFC from which he will give us copy 2. Is that right? Because as per IMM 3999, the DMP needs to give us copy 2 of 1017 EFC.

Please advise once more!!

Yes all you need to do is fill out the 1 page form on appendix C and bring it to the DMP, they will have the other form that you need. Also you will need to bring your pictures and ID (I used my passport and driver's license for identification). Then you keep co
py 2 which is green and should have one of you pictures atached to it with a maple leave stamp over it. You must submit this copy with your application. If it is lost you have to redo the medical. I would Scan the copy before you mail it so that you have a copy for your records.

We got a letter today from Buffalo, stating that medical results were received and to pay the RPRF if we haven't done so already and that an interview is not likely.

However, it also asked us to fill out the Statutory Declaration of Common-Law Union. I was pretty sure that, according to the instruction guide, we did not need to fill out this declaration unless a common-law partner was acting as a co-sponsor. That it was not necessary to fill out if one common-law partner is sponsoring another....

Mostly because, um, we sent in close to 300 pages of proof of our common-law relationship, and I fail to see how us checking little boxes on this form would in any way be preferable to the reams of information we had already sent... ah well. One more hoop to jump through.

Seriously, I just re-read the instructions for this form and it clearly states that we should not be filling it out in our situation. At least we know that our case is in process and being worked on, though, right??

I asked the same question, and yes, you do have to fill it in. Atleast they did not return the entire package to you!!

No worries, you can go to city hall (if in Canada) and have it signed there. no worries, and cheap to, I did it yesterday for $22.50.

well - both the document checklist AND the instruction guide state that the form is meant to be filled out if you are the sponsor and have a common-law partner acting as a co-sponsor. but whatever. it's easily done.

my one remaining question - am I (the sponsor) the principal declarant? or is my partner (the principal applicant)?

You don't have to fill it out. Its not needed for a common law sponsorship case only when a common law partner was acting as a co signer, although I've heard of them requesting the form later in the process. I guess each IO is different in what they want.

Don't know, but we used me as the declarant and my sponsor as spouse (if your asking about the declaration)

If you are asking about the complete package, your the sponsor, and your spouse the applicant...
(Hope I did not step in it there...)

Well, we finally did it.... my husband and I finished packing up our lives in Michigan and drove our Uhaul from Michigan through Indiana, Illinois, Wisconsin, Minnesota, and North Dakota, finally arriving at the border of North Dakota and Saskatchewan where my husband landed first thing on Tuesday morning. We drove away from the border and right in to the FREAKIN blizzard that hit the mid west. After nearly dumping our SUV in to the ditch along with the Uhaul we were pulling we had to turn around and get a hotel room for the night in Saskatoon. We left there this morning, worried that we werent going to get very far because the roads were very icy. BUT WE MADE IT... we arrived in Edmonton early this evening, got the Uhaul unpacked, reunited my husband with the daughter and the dog... and now we are ready to continue our marriage here in Canada... just in time to celebrate our 12th anniversary on the 30th...

Sometimes the doctor's office does not give you this form. I got my medical exam done in Montreal, Canada and they only gave me a receipt which they said was all I needed as proof. After I figured out that the Outland list requires the IMM 1017, I spent about 3 weeks calling the doctor's office trying to get it. In fact, I had to argue with one of the phone assistants, because she was insistent that doctors offices "never give it out and it is not necessary." She even tried to say it was confidential. I told her I knew others that had gotten it and that it is indeed on Immigration Canada's official required list of things to submit. I finally got someone in the office that knew what the form was and agreed to mail it to me. (And I got her name, and then only dealt with her for the rest of the process, but it still took a week or more to get her to send it to me.) I had to pay an additional $25 and give them two more photos. (They had only wanted 3 initially.) Anyhow....all that to say, I wish I had made sure I had gotten the 1017 when I was at the medical appointment, instead realizing later that I needed it (when I decided to apply Outland instead of Inland). And I think I probably had these had problems because perhaps most people getting the exam done in Canada are doing an Inland application and (I think, according to what I remember) the Inland process might only require the receipt as proof.
