It says on the form to attach a separate sheet of paper if there is not enough space. Write the name of the form(IMM 5669 Background Declaration) and question number on this separate sheet. Then on the form itself, you can write "see Addendum attached". This way the officer knows to look for the rest of the information on that sheet.
Now onto your dates below - they need the information from when to turned 18 yrs up to now, right?
You have gaps of approximately 1 month each in your dates - Example:
2005-08 to 2006-05 but you don't say what you were doing from 2006-05 to 2006-06. I think a better approach is to do it like this: (the ones in red are the gaps)
2005-08 to 2006-05
2006-05 to 2006-06
2006-06 to 2007-11
2007-11 to 2007-12
2007-12 to 2009-06
2009-06 to 2009-07
2009-07 to 2009-10
2009-10 to 2013-04
2013-04 to 2013-05
2013-05 to 2014-07
2014-07 to 2014-08
2014-08 to 2014-12
2014-12 to present
TIP: For the year when you turned 18, write what you were doing for that entire year, not just from your birthday month. This way, they will have no reason to request more information.
Provide details of what you were doing during the gaps noted above. It's fine if there is an overlap in what you doing (be it working, studying, travelling, unemployed, etc - just explain) but there can't be gaps. This is how we did my husband's sponsorship application and we got PPR + medicals last week.
