I am currently trying to apply to several graduate schools. However, I realized that there are difference in terms of surname/family name in documents.
In my passport my name is written as follows
BINSAR [MIDDLENAME] [SURNAME]
And in my college transcript it is written like this
BINSAR [MIDDLENAME]
When filling out my biographical data in the online admission page, I ran into an option that asks on whether or not I have former name(s) in my previous records and documents.
So, in my case, should I do choose yes, and then fill the former name field with that without the surname?
And should I make an affidavit letter in the future to declare that the names on my passport and my transcript are of the same person?
