super visa

Hi

I applied single entry super visa for my mom and she was granted visa from June 10th 2012 valid until June 10 2014. she came to Canada on June 13th, 2012. she is leaving on Sept 13, 2012 since my dad is alone there in India. Now i wanted to apply for Multile entry super visa for her. can i apply multiple entry super visa in Canada before she leaves to India.

2/and who is able and have right to apply for this visa ??
3/and what documents do i need to apply for it ?

My mom has 10 years valid super visa. Does she need the following

1. ETA
2. Medicals
3. Medical insurance from Canada

Please HELP ME
