Super Visa---Which Forms to Complete?

Hi
i am planning to apply for super visa for my parents.

Do they only fill the Main Application form which is called IMM 5257E

or

do they also fill in the Family Information - IMM5645, Schedule1 for Temporary Resident Visa, Statutory Declaration of Common Law Union - IMM5409.

Please help me.

Many thanks, PMM. Yes, my parents are married, so they will not complete the IMM-5409 Common Law Union form.

But will they still fill the Schedule 1 and Family Info form?

Assuming they will be applying at the New Delhi office,

CIC website has general information but more often than not, each visa office will have specific requirements. so always check the visa office website for those requirements.
