1) Put deceased, if this is an option
2) Do you know their year of death? Otherwise, is the an unknown option?
The options on the form are :

  1. Anulled Marriage
  2. Common-Law
  3. Divorced
  4. Legally seperated
  5. Married - Physically present
  6. Married - NOT Physically present
  7. Single
  8. Widowed

No, The issue is she has got 6 sisters and 1 brother and she doesn't remember their birth dates / year. The only option available in the form is the date picker. If i type anything else it says the format has to be YYYY-MM-DD

@canuck78 Thanks a lot for the response.
  • The reason is both my mom's mother and father is no more. In this scenario, we should mention widowed as martial status for both right ? Anyways in the present address field - i'm mentioning the "DECEASED" part.

  • What about the dates ?
So this will be answered like this:
Your moms mother and father - if your mom's father passed away first, he will be deceased while your mom's mom would be widowed until her death.

also to clarify, at your mom's father's death, if his wife was alive, his marital status would be married.

Since it appears that there are some issues with the online form, can you submit a paper application?

@sribrosrs - Yup i just got this info confirmed from a group in facebook as well. The last known martial status has to be updated.

Thanks All !

And for the dates part, (BIRTH & DEATH DATES) - The date of death was known. i simply did a reverse age caluclation and found the year of birth. The date and month were assumed.

Hi!

I am hoping someone can please help me in this forum. I am a Canadian citizen and would like to invite my father, through super visa, to come and visit me in Canada. I created a CIC account for him and completed the forms for him.

Questions:

1) Can I upload all the documentations for him and submit the super visa application even though I reside in Canada?
2) Can I pay his application and biometric fees using my credit card?

Appreciate any information from anyone!

Thank you so much.

Hi @canuck78 ,
Could you please check if you can help me with this query, I have checked on different forums and i have gotten mixed responses. If you could shed some light, that would be very helpful..

My parents native language is Punjabi but their passports are in English. In this case do they have to fill the names in family information form in both Punjabi and English?

@canuck78

For form 5645 Family information, for section B, - Children, should I include my information as well since it is stated in the form to "include ALL sons and daughters, regardless of age and place of residence? Thank you.

Hi All,
Need help for mom's supervisa. I am planning to apply for my mom's supervisa and have following questions. Will really appreciate your response.

1. My mom does not have any financial proof to show and no property and my father does not stay with her. But my mom stays with my 2 brothers who have jobs in india. How do I prove she has strong ties to home country?
I plan to bring her for 1 year to Canada and she will need to go back post that.

2. Is her marriage certificate really needed ? She has my father's name on her passport.

3. Will it be better to go through an immigration consultant to improve her chances?
Note: my income is above the LICO cutoff.

Please suggest.
