Super visa question Please help

My father is a passport is damaged and we are applying for his new passport.
We are going to apply for Super Visa soon ( previous application refused ).
My father's previous medical tests are still valid as they are under 1 year.
However, on the previous medical sheet, an old passport is mentioned. Now as we applying with a new passport (with updated new address ).
What should I need to know so that Immigration know the medical sheet attached is legitimate.
Please advise/help. Thank you

Thank you Canuck, does it means they will get to know through their system or also can we submit a self-explanatory letter ( where we can mention previous passport number and new passport number) ?

You don't need to submit a letter.
