super visa extention in Canada , upfront medical check require even done within less 12 months

I extended my father visa record in Canada last year less 12 months, now I try to extend online,but even i choose medical check less than 12 months.The final required documents still has upfront medical check? how come? so new even less 12 months ,every time extend the visa record need to do upfront medical check.that is ridiculous ....even my father all time is staying in Canada.
