Pretty sure the form doesn't say "you have been living in your country or territory of residence".

The dates From - To Under "Current Country or Territory of Residence" are validity start and end times, so enter when you permit started and when it expires. If you put your mouse over the to and from fields without clicking you should see a helpful little tip pop up.

With the passport, do I have to attach visa as well, right?

I know an immigration consultant who works with an immigration lawyer with 100% success so far in appeals. It can be length but when the reason for refusal seems unreasonable they chances of wining your appeal is high.
There is no lawyer with 100% sucess rate.

You need to come up with a plan because you are unlikely to get ab approval for September. You may get a refusal but there is a huge backlog of study permits that haven't been processed all the way back to March maybe even earlier.
