Study permit extension but college change

1st sem in college where I got admission for study and sem was not cleared. And on 2nd Sem I withdrawl from 1st college and took admission in another college and continued my further studies in different course .
So what things I need to do for doing my application for study permit extension.

As my study permit going to expire in 2 months and my 1st sem is cleared in second college.
