Sir, I cannot believe a PhD student COULD NOT figure out that on the official site. You are a RESEARCHER, for God sake.

You may change your school, program and/or field of study without applying for a new study permit. You do not need to apply for a change to the condition of your study permit, either. You may continue to use your study permit as long as it is valid.

Concerning you second question

Sometimes you need to have also some papers from your school because just to have a valid SP may be not enough, you have to be ENROLLED into program.

Be also aware that if the new rules come into force on June 1, 2014 you may not be allowed to enter Canada because according to the new rules your study permit expires 90 days after you dropped your first course. (if the rules will be retroactive. And I believe they will be.)
