Since you would probably be their representative and doing the uploading at the myCIC, it's easier for you to go to a Canadian notary instead.

Hi,

I am planning to get upfront medical exam for my mother’s Super Visa application.

According to the IRCC instructions, "When you have an upfront medical exam, the physician will ask you to fill out an IMM 1017B Upfront Medical Report. You must include a copy of the report with your paper application. If you apply online, you must upload the report before you can submit your application."

Does the physician provide the IMM 1017B report form to the applicant? If yes, can I upload it in the "Other Documents" section under their Super Visa application?

Do I also raise a web form as soon as I submit the application as others have done the same?
