Hello, for common law requirements it says
a minimum of two statutory declarations from individuals with personal knowledge of your
relationship supporting your claim that the relationship is genuine and continuing

question: can this be a fill out form rather than a written letter?

For statutory declaration, here is a form. You can make it with your computer, print out and get
signatures. It's the same as a letter, but if your family or friends write in different language from English, it will be required to be notarized.

quote:

STATUTORY DECLARATION

CANADA
PROVINCE OF ONTARIO
TO WIT:
}

In the matter of the relationship between [ ] and [ ]

I, [full name] of [full address] in the Province of Ontario, Canada,

do solemnly declare as follows:

1.That the common-law relationship between [ ], and her partner, [ ], is known to me.

2.That said relationship is genuine and continuing.

3.[Other details]

4.[Other details]

And I make this solemn declaration conscientiously believing it to be true and knowing that it is of the same force and effect as if made under oath.

Signature of declarant ________________________________

DECLARED before me at (City, Town Village) _______________________________

in the province of Ontario in Canada

I think you can use that and notarize it , you need to indicate their relationship to you , how do they know the sponsor /PA, and how long they know of your relationship .
I used Un-notarized letters from friends and family members , wasn't required by my VO but I sent them anyway .

You can use something like that if you want but there is no requirement for the letters to be so formal. I believe it is better to have people write simple letters in their own words. It should describe how they know you, how they met your partner and maybe a specific event where they saw you guys as a couple. They need to include that your relationship is genuine and ongoing.

You could follow that form, but note that it is actually not much of a form, since the answers to 3 and 4 will vary widely. However, basically that is what should be written. It is best for the letters to be in the person's own words, because if they look too much alike the visa officer might think you wrote it yourself, or that you told them what to say.

Some lawyers would follow that form or one like it for an affidavit. Just make sure the answers to 3 and 4 are detailed and personal, and seem to have been actually written by the people themselves.

Ok thanks all for your input. I think now it may be better to just have them write simple letters.

Also consider it is better to have them write letters in their own words and writing style, this will make the letters look more genuine.
