Is there anyone who has recently requested "state police certificate" from massachusetts. I tried going to http://www.mass.gov and follow the steps but I am still confused which form do I have to send and do I need fingerprint or not.

u need submit one form with basic info for Mass clearance and also have to send MO. Even i had trouble filling it online, so i send it thru post to Mass.
