SPOUSE Sponsorship-Request from the Embassy Appendix A:I'm Confused. Please Help

I received an Email from the Embassy with the following requests:

In order to continue processing your application, we require the following information and/or
documentation:

 Fully Completed Appendix A (attached) for you and each of your family members, whether
accompanying you to Canada or not. Passport details are not required for any family member who
is not accompanying you to Canada.

 Original Passport(s) for you and each family member who will accompany you to Canada
To ensure timely processing and facilitate your travel, we strongly recommend that you submit
the new red-coloured passports currently being issued by the Philippines authorities as they are
in compliance with the machine-readable passport Requirement of the International Civil
Aviation Organization.

 Original NSO Advisory on Marriages for yourself

My questions are:

1. In Appendix A form, am I going to include information about my parents and siblings? (My husband is the one who sponsored me. We don't have any children yet).

2. About the NSO Advisory on Marriages, is it the same with the NSO Marriage Certificate?

and lastly,

3. Included on the Email is the Appendix B - Photograph Specification, but the requests i noted didn't ask for any copies of visa photographs, am I going to include some copies upon submission?

Family members almost always means: spouse and children. Siblings and parents are only mentioned in the additional family form.

If they didn't ask for photos, then you don't need them. If it makes you feel better, you could probably include some anyway.

I have no idea about the NSO thing. Sorry.

hi march2014, NSO Advisory on Marriages (AOM) is different from your marriage certificate. AOM shows how many marriages you had and the name of the spouse, where and when the marriage happened. CENOMAR is also different, this is a Certificate of No Marriage. It shows that you are single and never been married thus you can contract marriage. When you fill up a form request at NSO, specify you`re requesting an AOM.

To add to that, when I called up on the NSO Hotline to request for an AOM, the rep said, that when you use the online facility, when you want to get an AOM, you still would use the CENOMAR option. They would check their database for past records of marriages and even if you request for a CENOMAR but you indeed have a record of marriage in the NSO database, they would issue you an AOM.

for #3. did you include pictures as well?
There is also appendix B in my email but it doesn't include pictures on the reqiurement list:

· Fully Completed Appendix A
· Original Passport
. Payment of Right of Permanent Residence Fee (RPRF)
· Original NSO Certificate of No Marriages (CENOMAR) for yourself
· Marriage search results for your sponsor from Vital Statistics Canada
