Spouse Sponsor forms Help Needed Please

Should I write the Sponsor Name & applicant name on each form? and How should I sort it in the final package.

Any help will be much appreciated..

Are you talking about attached information on a separate piece of paper? because if your not i have no idea what your asking .If yes then yes put the sponsors name and date of birth and the question # you are referring too.

All CIC forms automatically have your name on top right of each page if you filled up the online forms. Other supporting documents, I usually put applicant's name..sponsor's name on each pages and putting numbers on each pages just in case they got lost ( example: page 1 of 6 ..etc)

Thanks for your replies. Yes I am asking about the additional documents that I am sending. (i.e. police reports, Phone logs, etc)
