Spouse open work permit for March 2018

my file is went into Review today, but the Medical has not been updated? when they usually update the medical? I have done upfront medical. Please suggest.

I am writing on this forum to have more information if possible about my on going work permit application.
First of all, I am a provincial nominee from New Brunswick. My spouse and myself have obtained a job offer from an employer in that province.My spouse and I have made a work permit application on Jan 2019 but unfortunately on March 2019 I got a refusal from Ircc.
I decided to reapply again for a work permit and my spouse apply for an open work permit on the 1st June 2019.
On 4th June 2019, we receive our AOR for our PR application from IRCC, Nova scotia.
The processing time for work permit application is normally 9 weeks. On 22 August 2019, we got a request letter from IRCC in pretoria to deliver a police certificate to continue processing our application. expiry date for delivery was the 22 sep 2019.
on 12 Sep 2019, we submitted both our police certificate (file was well submitted) and till today we are still waiting for an answer from IRCC.
Does anyone knows how much time will it take for us to get a reply and will that be an approval this time?
