Have you ordered notes? There's no way to say really. 12 months is quite long for a US application, but you mention you could not get a medical exam for a dependant child, so that is probably why it's taking longer. Anything "out of the norm" will cause delays.

Not directly, they do not look at credit or loans. You just have to show how you can support yourselves.

Only the sponsored person (applicant) needs to fill this out, the sponsor's name just goes in the "spouse" box. The country specific checklist is the PDF under Part 3: Information guides.
